Open Door Pathway Foundation works alongside trusted community partners who serve adults and veterans in transition. Our partners help identify, refer, and support residents as they move into stable independent shared housing.

How It Works
Simple, respectful, and confidential
Our process is designed to make collaboration easy for busy partners while ensuring every resident is placed thoughtfully.
Partners reach out by phone, by email at partners@OpenDoorPathway.org, or through the referral form to begin a conversation about a prospective resident.
Partners submit a brief referral with the candidate's basic information, history, and housing needs.
The Foundation completes screening, interview, and placement matching in coordination with the referring partner.
Partners remain a trusted point of contact for residents, with the Foundation providing transparent updates and communication.
