How It Works
Referring a client to Open Door Pathway Foundation is simple, respectful, and confidential. Our process is designed to make collaboration easy for busy partners while ensuring every resident is placed thoughtfully.
Four-Step Process:

Partners reach out by phone, email: (partners@opendoorpathway.org), or referral form to begin a conversation about a prospective resident.

Partners submit a brief referral with the candidate's basic information, history, and readiness for independent living.

The Foundation completes screening, interview, and placement matching in coordination with the referring partner.

Partners remain a trusted point of contact for residents, with the Foundation providing transparent updates and communication.
